Informed patient Consent for Aligners Globe

How do I take care of my Aligners?

Read the instructions carefully which you have to follow to get the perfect smile you dreamt of.

Insertion and Removal

• Aligner trays ONLY work when you are wearing them. We recommend you to wear each Aligner as instructed by your Aligners Globe provider, FULL TIME a day and night, at least 22 hours for 15 days(MINIMUM of two weeks), and wear all trays sequence-wise and complete your box in this way. The only time the aligners should be out of your mouth while having food, snacks, brushing, flossing, and cleaning trays and drinks because food and drinks get stuck under the trays.

• Make sure you have the proper tray number, also keep in mind U is for upper and L is for lower teeth. You should wear them in proper sequence. They are labeled very small on each aligner.

• When inserting the trays into teeth, first push them gently over the front teeth. Then, apply balanced pressure on the top of the left and right molars until they snap into place. DO NOT bite your aligners to seat them because they will crack or might even break.

• Make sure each aligner tray is ALL THE WAY DOWN and FULLY SEATED. You should not see any space between the edge of the teeth and the edge of the aligner tray. If the aligner tray is not fully in place, the teeth will not move properly and the next aligner will not seat. This can waste many weeks of treatment and MONEY, starting over with possible extra fees or even sometimes you have to purchase a new Aligners Globe box.

• For removing the aligner trays from the teeth, apply balanced pressure in the premolar area on both sides of the tray using index fingers or Aligner removing tool, then gently lift off the front teeth. Always rinse the aligners in cold water and store them in the container provided. To prevent damage to trays, avoid unnecessary removal. DO NOT use sharp objects like toothpicks to remove your aligners. Always use the aligners removal tool provided in the aligners globe box to remove the trays if you are unable to remove them with fingers nails like some female patients have acrylic nails extensions and you will feel difficulty in the first three days of each tray as they will feel tight in removing.

• At the start you might feel a slight difficulty in speaking and may have increased salivation and sometimes dryness as your tongue familiarizes, this goes away just in a few days. 

• If you feel any sharp edge hurting your gums while having the tray on, simply remove the tray and file the sharp edge away with the filer tool provided in the aligners globe box. If of filing problem exists then immediately inform your Aligners globe provider for an appointment as your dentist will fix it in the office.

• Initially, on wearing the new or first tray, Aligner fits tightly and you may feel some pressure which is normal and is required to move the teeth. It is very important that you NEVER go to the next aligner before the instructed time (15 days). Even though it may seem to you that the current aligner is loose and no longer putting pressure on teeth, bone maturation is taking place and roots are changing their direction. Most of the tightness will go away at the end of two weeks but if the aligners are still too tight then inform your Aligners globe provider immediately.

What to eat and drink while having Aligners?

• Except for cold or room temperature water, always remove aligners while drinking anything other than water. Do not drink any hot, fizzy liquids like tea coffee, coke, or ice cream while aligners inside your mouth. Any other fluid will get inside the aligner and be held against the teeth possibly for hours and can cause staining or damage (cavity) to the teeth and aligners which means you can only drink water while having Aligners in the mouth. 

• Brush and floss normally with the aligners out. It is important that you brush your teeth after every meal and before re-inserting your aligners to maintain necessary hygiene. Large food particles may interfere with proper insertion and may lead to halitosis( bad breath)

• There are no food restrictions with Aligners. However, aligners must ALWAYS be removed when eating.

• DO NOT use chewing gum while wearing aligners.

 It is advised NOT to smoke while you have aligners in your mouth as it will stain the aligners' trays and teeth.

• If you have any trouble with your treatment, please call your Aligners globe provider.

Care of Aligners Trays

Handle your Aligners trays with care as they are breakable and their replacement is Expensive.

• Always Store Aligners only in the provided Aligners case. Never place aligners on the side/food table or napkins and any other case because as these are invisible trays you might lose them. Never place it in a pants pocket that can be sat on.

• Clean your Aligners with soft bristles toothbrush and cold water, DO NOT use toothpaste. If you don’t have access to a toothbrush or forget to carry an orthodontic kit along with you in school/ office/ restaurant, etc., simply rinse your mouth and rinse aligners trays under running normal water and wear it again and remember to clear them with a toothbrush with early convenience.

• You can soak aligner trays in transparent mouthwash and dispensing cleaning tablets.

• Do not keep the Aligner box/Aligner where the pet animals and the children can reach them.

• Do not let anyone else in your friends or families try on your Aligner because every individual has different jaws and teeth size and misalignment issues.

• Do not place aligners in boiling or hot water to clean them, as they will deform and damage. Take out aligners from the mouth while having hot, sticky, and colored liquids. Hot liquids can deform the trays. Aligners trays should be kept away from the heater, dishwasher, car dashboard, or any place where the temperature exceeds 115° (F°). 

• If you miss some days for any reason, (wedding, illness, pregnancy, exams, etc.) pick up where you left off and wear the aligners for a full 15 days MINIMUM. Call the aligners provider to let them know otherwise Aligners Globe will NOT consider any excuses.

• Always bring your Complete Aligners Globe box including All old/previous trays either broken or new ones, when you visit for follow-up appointments. NEVER DISCARD any old aligner trays whether it is broken or not, you have to keep them safe in the aligners globe box otherwise you are unable to claim their warranty.

• Call Aligners Globe provider IMMEDIATLEY, if you lose or break an aligner, and start wearing the previous aligner if available. Too much time without your Aligners may cause relapse( teeth shifting back to original position)

• There will be a charge for damaged or lost aligners.

• Regularly visit the treating doctor for evaluation of the ongoing treatment process.

 Travelling Guide 

• If you plan to travel for vacations, even if you are normally good at keeping track of your items, you never know what can happen on a trip. At a minimum, you should bring your previous set, current set, and upcoming sets of aligners with you on your trip.  

• If you are flying, you should keep your aligner trays and container box in your carriage bag. Cargo Baggage can easily be lost/manhandled and you don’t want to go for days without wearing your aligners. When you are on the road or in an airplane, it can be difficult to brush your teeth every time after meals. At the very least, make sure you clean your mouth and your aligners before placing them back in after eating. 

A dry mouth with aligners is a common observation, but an irritating condition. As a result, many patients crave something that will keep their mouth fresh and healthy. Some patients try sucking on sugar-free xylitol mints to address this issue.

Retainers Period

When your smile journey with Aligners Globe has ended. Your teeth are straightened, your smile is worth showing off and the bite is in its exact place. You felt relieved and excited. Once your orthodontic treatment of teeth alignment is completed, the risk of relapse exists. Relapse is the loss of any corrections achieved after orthodontic treatment.

As the name indicates, it “retains” the movement of teeth in new positions achieved by your braces or Aligners Globe. Retainers are of two types, fixed retainers, and removable retainers. Your orthodontist (Aligners Globe) Provider will decide which type of retainer will suit you according to your case.

Removable Retainer Time duration: 2 years

Fixed or permanent retainer Time duration: 2 years or  fixed on teeth rest of Life 

Wearing a dental retainer after any orthodontic treatment is an extremely important step because it keeps teeth stable in their new positions. If you want your teeth to remain in their ideal new position, it is best to wear the dental retainer as instructed by your orthodontist -Aligners Globe Provider.

Always ask your orthodontist IMMEDIATELY to get you an extra set of the removable retainer in case you break or lose the first one to avoid relapse.

We provide our patients with two pairs of removable retainers that cost-free.  and it is included in the Aligners Globe box,  however, you will be charged for the 3rd set of retainers. Your orthodontist will tell you the 3rd set of retainer price or permanent retainers cost.

Warranty Claim

Terms and conditions

Aligners Globe box is a treatment with a warranty. Results shown in the treatment Plan are guaranteed if the patient takes all measures and follows all above said instructions. We will ONLY Accept warranty in THREE conditions.

1. A patient has completed his/her smile journey in a given frame of time (we will not accept even a single day up). 

2. The last trays are fully seated on the teeth just like the first tray on the teeth which we have taken a picture of and kept in our record.

3. A patient has to keep all USED trays back in the box even broken trays (up to 3 broken trays are acceptable). In case of Missing of any single tray or broken tray pieces, from the Aligners Globe box, the warranty shall not be applicable. 

Acknowledgment

Aligners Globe provider/ Aligners Globe Certified dentist has discussed with me the reason for Aligner Globe treatment, its benefits,  potential problems related to recuperation, and the possible results of not having this treatment. This authorization is given with the understanding that any procedure involves some risks.  They explained to me how to wear and remove aligner trays from teeth and what is the proper sitting position on the teeth if I will not follow the guidelines I will be responsible for the results. They have also shown me complete fitted trays in my teeth and took pictures of me wearing first aligners fitted trays and kept them in their record. 

What my Aligners globe provider has handed over to me?

I have received  Upper -----Lower----- trays OR  complete Aligners Globe  box---------/ with 2 pairs of retainers---------  starting date-------- and ending date------------ of smile journey, and I understand all the instructions given by Aligners globe provider.

5. Retainer box

 I have been made aware of certain risks that are associated with this particular procedure. I understand that these risks can be serious if I will not follow the Aligner's care and aligners provider's instruction in the form of paying again the full payment of the new aligners box.  It has been explained to me that sometimes other procedures and conditions develop in the course of procedures discussed above which cannot be reasonably foreseen. It is also possible that my dentist may discover different unsuspected conditions at the time of the procedure. I do authorize the above-named dentist and his assistants or designees to perform the unforeseen procedure that is necessary for me according to their best medical judgment. I give my consent for observers to be present and for photographs for medical purposes to be taken.

I have had sufficient opportunity to discuss my condition with my dentist and his associates and all my questions have been answered to my satisfaction. I understand this consent form fully.
